Few reminders about your appointment:

• Authorization for the new patients and our current patients are required. Please bring your ID and Insurance card with you for your appointment.

• Please plan to arrive 15 minutes earlier to fill out new patient paperwork!

• Please bring in your personal health documents as they may be needed.
  
• Parking is available under the clinic building.

• Please understand that if you are more than 15 minutes late for your appointment, we may have to reschedule.

• Co-pays and deductibles are due at the time of service.
